If you currently receive supports through Centacare and your supports are transitioning to Neta Care,
we want this to feel like continuity, not change.
What's Happening
A planned transition.
Neta Care has been named as the partner provider for clients transitioning from Centacare's NDIS-funded supports. The transition is being coordinated with Centacare to keep continuity of care, retain familiar relationships where possible, and minimise disruption to your routine.
This is not a takeover. It is a handover — and one that you and your support coordinator stay in control of.
What Stays the Same
Continuity is the priority.
Your support team
Where possible, the same support professionals continue to provide your supports — they transfer their employment to Neta Care.
Your schedule
Your existing shift times, frequency and pattern of supports continue as agreed. No need to relearn a new routine.
Your goals
The goals and plans you set with Centacare continue. We pick up where they leave off, with the same focus and intent.
What Changes
The few things that need updating.
Service agreement. A new service agreement is required between you and Neta Care. We send this to you for review and signature — your support coordinator can help.
Invoicing and claims. Invoices for shifts after the transition date come from Neta Care, not Centacare. If you are plan-managed, your plan manager is informed of the change.
Contact details. Your contact point becomes the Neta Care intake team and your allocated coordinator. Phone, email and after-hours numbers are provided in your welcome pack.
Your Transition Steps
A five-step path.
1. Welcome call. We introduce ourselves, confirm your supports and answer questions.
2. Service agreement. Review and sign a service agreement with Neta Care.
3. Plan handover. With your consent, your support plan and clinical notes transfer from Centacare to Neta Care.
4. First Neta Care shift. Same support professional where possible, on your usual schedule.
5. Check-in. A 30-day check-in to confirm everything is on track and address anything not working.
