A coordinated approach to care.
GPs & Primary Care
Care continuity across primary, allied and community health. Clear reporting on participant progress, prompt response to your referrals.
Specialists
Geriatricians, psychiatrists, rehabilitation specialists, paediatricians and others. We coordinate around the specialist plan.
Hospitals & Discharge Teams
Rapid intake for discharge planning. We are practised at picking up care quickly when a participant is leaving an acute setting.
Allied Health (External)
Coordinated multi-disciplinary care for shared clients. Letters, reports and case conferences as required.
Community Health Services
NGO and community sector partners working with shared client populations.
Aged Care Facilities
Where a resident is discharged or supported in the community, we coordinate with the facility care team.

Sending a Referral
Send referrals to [email protected] or call our intake team on 1300 389 562. Include:.
• Participant name and contact details
• Plan management type and funding source
• Brief overview of support needs and clinical considerations
• Region where services are needed
• Consent to contact participant or family
Active Transition: Centacare to Neta Care.
If you have referred clients to Centacare in the past, the transition pathway is now open. Continuity of care is the priority.
