Wound care
Pressure injury management, post-surgical wounds, chronic ulcers, complex dressings.
Medication management
Webster pack reviews, injectable medications, complex regimens, education for participants and families.
Continence care
Assessment, product selection, catheter care, stoma care, bladder and bowel programs.
Post-acute care
Hospital discharge support, IV therapy continuation, recovery monitoring.
Complex care
Tracheostomy care, enteral feeding, ventilation support, palliative care nursing.
Health monitoring
Vital signs, diabetes monitoring, chronic disease management, GP liaison.

For people who would otherwise need to attend a clinic, transfer to residential care or remain in hospital longer than necessary, community nursing delivers the clinical care at home. Better for recovery. Better for dignity. Better for families.
Our nurses work alongside GPs, specialists and allied health to keep care coordinated and discharge plans actually working.
Funding
How nursing care is funded.
NDIS. Under Core Supports — Assistance with Daily Personal Activities (high-intensity), where a participant's needs require nurse-delivered care.
Aged Care. Through Home Care Packages and CHSP allied health.
DVA. Community Nursing programme for Gold and White Card holders.
Medicare. Some nursing services with GP referral under chronic disease management plans.
